Medicare Advantage vs. Supplement: Which Plan Is Right for You?

One of the most common questions I hear from seniors approaching Medicare eligibility is: "Jorge, what's the difference between Medicare Advantage and Medicare Supplement — and which one should I pick?"

It's a great question, and the honest answer is: it depends on your health, your lifestyle, and your budget. Let me break it down in plain language so you can make a confident decision.

What Is Original Medicare?

Before we compare the two options, let's start at the foundation. Original Medicare is the federal health insurance program for people 65 and older (and some younger people with disabilities). It has two main parts:

  • Part A covers hospital stays, skilled nursing facility care, and some home health services.
  • Part B covers doctor visits, outpatient care, preventive services, and medical equipment.

Here's the catch: Original Medicare doesn't cover everything. You'll still face deductibles, copays, and coinsurance — and there's no cap on out-of-pocket costs. That's where Medicare Advantage and Medicare Supplement come in.

What Is Medicare Advantage (Part C)?

Medicare Advantage is an alternative way to receive your Medicare benefits. Instead of getting coverage directly from the federal government, you enroll in a private insurance plan that is approved by Medicare.

These plans must cover everything Original Medicare covers, but most also include extra benefits like:

  • Dental, vision, and hearing coverage
  • Prescription drug coverage (Part D)
  • Fitness memberships (like SilverSneakers)
  • Transportation to medical appointments
  • Over-the-counter allowances

Who Is Medicare Advantage Best For?

Medicare Advantage tends to work well for seniors who:

  • Want low or $0 monthly premiums (many plans have no additional premium beyond Part B)
  • Are generally healthy and don't expect frequent hospitalizations
  • Prefer having dental, vision, and hearing bundled in one plan
  • Live in an area with a strong network of in-network doctors and hospitals
  • Are comfortable with HMO or PPO network restrictions

The Trade-Off

With Medicare Advantage, you typically need to use in-network providers and may need referrals to see specialists. If you travel frequently or split time between states, network restrictions can be a challenge.

What Is Medicare Supplement (Medigap)?

Medicare Supplement, also called Medigap, works alongside Original Medicare. You keep Original Medicare (Parts A and B), and your Medigap plan pays for some or all of the costs that Original Medicare doesn't cover — like deductibles, copays, and coinsurance.

There are standardized Medigap plans labeled by letters (Plan G, Plan N, etc.). The most popular is Plan G, which covers nearly all out-of-pocket costs after the Part B deductible.

Who Is Medicare Supplement Best For?

Medicare Supplement tends to work well for seniors who:

  • Want predictable, low out-of-pocket costs throughout the year
  • See specialists frequently or have ongoing health conditions
  • Travel often or spend time in multiple states (Medigap works with any doctor who accepts Medicare nationwide)
  • Value the freedom to see any Medicare-accepting provider without referrals
  • Are willing to pay a higher monthly premium for peace of mind

The Trade-Off

Medigap plans typically have higher monthly premiums than Medicare Advantage plans. They also don't include prescription drug coverage — you'll need to enroll in a separate Part D plan for medications.

The Bottom Line

There is no universally "better" plan — only the plan that's right for you. Here's a simple way to think about it:

  • Choose Medicare Advantage if you want low premiums, extra benefits, and are comfortable with a network.
  • Choose Medicare Supplement if you want maximum freedom, predictable costs, and see doctors frequently.

The most important thing is to compare your actual options in your area — plan availability and costs vary significantly by ZIP code.
